Third Screen Enter the first & last name, email of the person taking the exam, check yes if you have read the study guide, enter the last 4 digits on Soc Sec #, enter the dentist’s name, check yes if worked 3 months, or check no if moving to SC from another state moving from, enter the name of the state moving from. Click Start
Pay $60.00 continue
Do not log into Paypal *Pay with a credit or debit card. ENTER
Complete information for the name on the Credit or debit card only. ENTER
Un-check the box “Save this information with a paypal account”
Enter security Challenge (code they give you). Continue
Again Do not login, Continue without logging in
If you continue any further you will be logging into the exam with your email address and your password you created.
